Unsafe doses leave the cabinet on override
On override, a nurse pulls a medication before any pharmacist reviews the order — and a wrong-drug pull can be fatal. In the widely reported 2017 Vanderbilt case, an override withdrew the paralytic vecuronium in place of the sedative Versed, and the patient died. ISMP targets no more than 5% of withdrawals on override; real-world units run well above it.
